WINTER Sports Clearance Update
Complete sports clearance packets will be
available at CHS, OHS, and the district Athletic Offices beginning November 3rd
Clearance Dates:
Capital High School:
-- The ASB Office is open weekdays to receive and process clearance for all fall sports beginning November 3rd and concluding November 14th (GYMNASTICS MUST CLEAR NO LATER THAN FRIDAY NOV. 7th). Office hours are 8:00 am to 3:00 pm.
Olympia High School:
-- The ASB/Athletic Office will be open for clearance on the following sports specific schedule.*
November 3rd Gymnastics
November 4-5 Boys Basketball
November 6-7 Boys Swimming and Wrestling
November 10 Girls Basketball
November 12 -13 Open to all sports
*if students do not complete the clearance process on the assigned day they may not be eligible for the first day of practice--November 17 (Gymnastics November 10)
Required Physical Forms
Physical Examination --initial and update--may be downloaded from the District website along with "Sports Specific Liability Waiver Forms" and "Transportation Forms". These forms may be turned in at the time of clearance.
Participation Fee
The Olympia School District Board of Directors recently approved an increase in seasonal participation fees to $125.
Athletic Clearance Process
All students wishing to participate in a school athletic program must submit a completed clearance packet prior to any participation. Failure to submit a completed packet could result in loosing eligibility.
Students will need the following:
- Athletic Registration Folder
- Athletic Participation Contract
- Physical Examination Form (only original forms and signatures will be accepted)
- Travel/Emergency Medical Card
- Sport Specific Liability/Safety Guidelines Forms
- Student Driver/Rider Form (if applicable)
- Purchase of an ASB card
- Pay all "Participation Fees -- $125
- Clear or pay all fine
